Likelihood and consequence
Risk has two parts. How likely is harm? And how bad would it be? Even a small chance of someone dying is a high risk.
The hierarchy of control
Start at the top of the list. Stop as high up as you can. The top controls work best.
- Eliminate — do not do the task that way at all
- Substitute — use a safer method or material
- Isolate — barricade, lock out, separate people from the hazard
- Engineering — shoring, guarding, mechanical lifting
- Administrative — permits, training, signage, rotation
- PPE — last, never first
